Full Time – Nurse Practitioner or Physician Assistant

Job Description

Full-time, in-person nurse practitioner or physician position focused on providing primary care in skilled nursing facilities.

Responsibilities:

• Manage clinical care and rounding at assigned SNFs
• Prescribe medications and treatments, ensuring timely documentation
• Provide patient-centered primary care and be available for consultations
• Ensure punctual attendance for scheduled shifts, with reliable transportation required

Shift hours are 7AM-1PM, Monday through Friday, with charting possible off-site. Compensation includes salary, productivity bonuses, and benefits, all within a supportive, growth-oriented environment.
