Job Description

Seeking a full-time Nurse Practitioner or Physician Assistant to provide patient-centered care in skilled nursing and long-term care facilities in Sequatchie, TN.

  • Serve as primary care provider, performing assessments and managing acute and chronic conditions to promote recovery and prevent hospitalizations.
  • Collaborate with healthcare teams, document care plans accurately, and ensure regulatory compliance.
  • Educate patients and families about health conditions and treatment options.
  • Complete Medicare recertification visits and documentation.

    • Qualifications include a Master’s or Doctoral degree, current Tennessee license, national certification, and DEA registration.
