Seeking a Licensed Professional Counselor to join a long-term care clinical team providing therapy, assessments, and behavioral support to residents in LTC facilities.

As part of an interdisciplinary team, you will deliver individual, family, and group therapy, support residents with common LTC conditions, and collaborate with facility staff to improve outcomes.

Responsibilities include documentation, care planning, and building strong relationships with facility teams.
