Job Description

The Wound Care Nurse Practitioner (NP), supervised by the Director of Nursing and overseeing nurses and medical assistants, provides specialized wound care in home, nursing home, and assisted living settings.

They perform assessments, develop care plans, educate patients and caregivers, and collaborate with healthcare teams to promote healing.

Qualifications include a Master’s or Doctoral degree in Nursing, current NP licensure, wound care certification within 6 months, and at least 2 years of relevant clinical experience.

Physical demands involve mobility, lifting, and environmental adaptability. The role requires travel, a valid driver’s license, and adherence to safety and infection control protocols.

Key duties encompass wound assessment, treatment, supervision of staff, documentation, and coordination with care teams. The NP must demonstrate strong clinical, communication, and interpersonal skills, with the ability to work independently and flexibly across various settings.
