Job Description
The Wound Care Nurse Practitioner reports to the Director of Nursing and supervises nurses and medical assistants.
Provides specialized wound care in home, nursing home, and assisted living settings for chronic wounds, pressure injuries, surgical wounds, and complex skin conditions. Performs assessments, develops care plans, educates patients and caregivers, and collaborates with healthcare teams to promote healing. Requires Master’s or Doctoral degree in Nursing, active NP license, and wound care certification within 6 months. Minimum 2 years of healthcare experience is preferred. Field-based role involving travel, physical effort, and adherence to safety protocols. Must have reliable transportation and a valid driver’s license. Key responsibilities include wound assessment, treatment, supervision of staff, documentation, and coordination of care, ensuring compliance with policies and HIPAA.
