Job Description

The Wound Care Nurse Practitioner reports to the Director of Nursing and supervises nurses and medical assistants.

  • Provides specialized wound care to patients in home, nursing homes, and assisted living facilities.
  • Performs assessments, develops care plans, educates patients/caregivers, and collaborates with healthcare teams to promote healing.
  • Requires Master’s/Doctoral degree in Nursing, NP licensure, wound care certification within 6 months, and minimum 2 years of clinical experience.
  • Must be physically capable of fieldwork, including lifting and mobility, with reliable transportation.
  • Field-based role involves travel, adherence to safety protocols, and exposure to biohazards.
  • Responsibilities include wound assessment, treatment, supervision of staff, documentation, and coordination of care.
  • Strong assessment, communication, and adaptability skills are essential.
