Job Description

*Overview*

We are seeking a compassionate and motivated *Speech-Language Pathologist (SLP)* to join our rehabilitation team in a Skilled Nursing Facility (SNF) / Post-Acute Care setting in Santa Rosa, CA. The SLP will assess, diagnose, and treat residents with speech, language, cognitive-communication, and swallowing disorders. This role plays a key part in improving residents’ safety, quality of life, and functional independence in a medically complex setting.

*Key Responsibilities*

*Clinical Evaluation & Treatment*

* Conduct comprehensive evaluations of speech, language, cognitive-communication, and swallowing functions.
* Develop and implement individualized treatment plans for residents recovering from illness, injury, or surgery.
* Provide evidence-based therapy, including dysphagia management, communication therapy, voice treatment, and cognitive rehab.
* Perform Modified Barium Swallow (MBS) or Fiberoptic Endoscopic Evaluation of Swallowing (FEES) if credentialed (optional).
* Recommend appropriate diet textures, compensatory strategies, and therapeutic interventions for safe swallowing.

*Documentation & Regulatory Compliance*

* Complete timely and accurate documentation in compliance with SNF, Medicare, and state regulatory requirements.
* Prepare evaluations, progress notes, care plans, and discharge summaries.
* Track and document minutes for PDPM/Medicare guidelines.
* Maintain confidentiality and adhere to professional and facility-specific standards.

*Qualifications*

* Master’s degree in Speech-Language Pathology from an accredited program.
* Current *California SLP License* (or eligibility to obtain).
* Certificate of Clinical Competence (CCC-SLP) preferred; CFY candidates may be considered depending on facility.
* Experience in SNF, post-acute care, or medical settings preferred.
* Strong clinical reasoning, communication, and documentation skills.
