Job Description
The Wound Care Nurse Practitioner treats patients in various markets throughout Southern California to provide and manage advanced wound care to patients who are homebound or in long-term facilities.
The Candidate must be:
1. A graduate from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) Program
2. Have an Active License to practice Medicine as an NP or PA with a current valid California license
3. Board Certified and provide evidence of Board Certification
4. Must have prior Wound Care experience and/or have a Wound Care Certification
5. Having specialty experience with any of the following: hospice, home health, palliative care, congregate care, and/or assisted living facilities is preferred but not required.
The Candidate must:
1. Be willing and able to travel throughout southern California to see patients, and receive training as needed
2. Provide a patient-centric approach
3. Have excellent verbal and written communication skills in a professional environment
4. Provide detailed and well documented progress notes within 24 hours of visit to Healing Wounds administrative Team
5. Be proficient with medical instruments and equipment required by Healing Wounds
6. Ability to communicate effectively, in verbal and written form, and be able to build rapport with clients and families, and other professional staff when serving clients
7. Have a sound understanding of all federal and state regulations
8. Have ability to lift 50 lbs. or more
9. Have full range of mobility and must be able to reposition patients
10. Have a valid and unrestricted driver’s license
Candidate must:
1. Perform complete physical exams of patients
2. Select, order, and perform tests and procedures to best treat the patient
3. Perform wound care procedures as deemed necessary and appropriate for a plan of care in accordance with Healing Wounds and best practice(s) procedures
4. Define and document patient diagnoses
5. Recommendations for prescription of required medications and/or Surgical/Wound Care Dressings to facilitate wound healing and/or wound prevention.
6. Assure all patients are completely satisfied, by providing excellent patient care
7. Patient education and management
8. Debridement of wounds by various methods completed safely during the patient visit
9. Application of topical wound care and other wound management products
10. Notate findings and treatment course in-patient chart, including follow-up notes
11. Actively participate in departmental activities to improve training methods and achieve company goals
12. Travel to patients’ homes and/or long-term care facilities will be required
13. Other tasks as required for effective and high quality mobile wound care management
Pay: $200.00 – $500.00 per visit
As a mobile wound care provider, the Candidate will have to travel and treat patients that are homebound (in the personal home) or in long-term care facilities and must be able to travel to the patient’s location. Candidate has flexibility in scheduling the appointments with the client regarding days and times but must collaborate with the client to ensure the client is seen and treated timely and in accordance with the treatment plan.
