Job Description

We are seeking a dedicated Primary Care Functional Medicine Nurse Practitioner or Physician Assistant to deliver personalized, integrative primary care in locations across Oregon.

  • Provide patient care via office visits, telehealth, and same-day appointments, including assessments, diagnostics, and treatment planning.
  • Develop personalized plans incorporating lifestyle, supplements, hormone therapies, and prescriptions.
  • Educate and empower patients on preventive and functional medicine principles.
  • Collaborate with a multidisciplinary team and support clinic operations in a positive environment.
  • Maintain accurate documentation in the EHR system and participate in team initiatives.

    • Qualifications include active Oregon licensure, board certification, and relevant clinical experience.
