Job Description

We are seeking a full-time Home Care RN Clinical Manager to lead and supervise our home health care team, ensuring quality care aligns with agency standards and objectives.

  • Responsibly oversee team performance, monitor competencies, and ensure delivery of high-quality services.
  • Manage patient care through case evaluations, treatment plans, and ensuring compliance with documentation and regulations.
  • Provide clinical oversight, review documentation, and facilitate quality improvement activities.
  • Coordinate patient schedules and support the implementation of care models and procedures.

    • Required qualifications include at least 2 years of experience in a Medicare-certified home health agency, a valid Colorado RN license, and basic computer skills. Preferred candidates have 1+ year of home health RN experience and leadership background.
