Job Description

We are seeking a full-time Home Care RN Clinical Manager to lead and supervise our home health care team, ensuring high-quality patient care in accordance with agency standards and regulations.

  • Provides clinical oversight, monitors team performance, and ensures accurate documentation and compliance with Medicare and state requirements.
  • Manages patient cases, reviews plans of care, and facilitates care coordination to optimize outcomes.
  • Participates in quality improvement initiatives and ensures adherence to care models and procedures.
  • Requires at least 2 years of experience in a licensed home health agency, a Colorado RN license, and basic computer skills. Preferred experience includes senior leadership and home health RN roles.
