Job Description
Seeking a Home Health Speech Language Pathologist to assess and treat speech, language, social communication, cognitive-communication, and swallowing disorders through home visits. Responsibilities include conducting evaluations, developing and updating care plans, coordinating with healthcare teams, documenting patient progress, and educating patients and caregivers.
- Master’s degree in Speech-Language Pathology with active license (CCC).
- At least one year of experience as a licensed SLP, with home health documentation skills.
- Perform evaluations, re-evaluations, and complete necessary assessments and data.
- Provide patient education and support to promote independence.
