Outpatient Services Case Manager/Therapist – LCPC, LPC, LCSW, LMSW

Job Description

We are seeking a full-time Outpatient Program Therapist/Case Manager (LCPC, LPC, LCSW, LMSW, LMFT) to provide comprehensive social services and therapeutic support to patients.

Responsibilities include conducting assessments, developing treatment plans, facilitating group, family, and individual therapy, and discharge planning. The role involves working with a dedicated team in an engaging environment aimed at promoting patients’ emotional health.

Shift options include three 12-hour days or four 10-hour days, with hours varying from 9:00 a.m. to 9:00 p.m. or 10:30 a.m. to 9:00 p.m.

Qualifications include a Master’s Degree in Counseling, Social Work, or a related mental health field, valid licensure, and CPR and Crisis Prevention training (paid).

Compensation ranges from $32.00 to $42.00 per hour, with benefits such as 401(k), health and dental insurance, paid time off, tuition reimbursement, and more.
