Nurse Practitioner – Geriatric Primary Care (SNF, LTC, ALF)

Job Description

We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in Skilled Nursing Facilities, Long-Term Care centers, and Assisted Living Facilities.

Deliver patient assessments, manage acute and chronic conditions, and develop treatment plans.

Order diagnostics, prescribe medications, and educate patients, families, and staff.

Collaborate with care teams to ensure seamless, patient-centered care.

Maintain accurate documentation and support quality improvement initiatives.

Qualifications: Current MN NP license, Board Certification, with 1+ year experience preferred in geriatric or post-acute settings. Strong communication and commitment to patient-centered care are essential.

This full-time role offers a competitive salary ($100K–$140K), flexible scheduling, benefits including health, dental, vision, 401(k), CME allowance, and minimal on-call duties.
