Join a dedicated healthcare team providing exceptional post-acute patient care in community facilities. This Advanced Practice Provider role involves managing and coordinating patient care, conducting rounds, communicating with staff and physicians, and documenting visits efficiently. We offer comprehensive training, shadowing, and oversight by a Medical Director to support your success.

Responsibilities:

  • Deliver high-quality patient care and coordinate treatment plans
  • Communicate effectively with nursing staff and physicians
  • Conduct patient rounds and facilitate diagnostic and therapeutic procedures
  • Maintain accurate electronic medical records and manage billing documentation
  • Adhere to organizational policies and regulations

Qualifications:

  • Excellent communication and empathy skills
  • Self-motivated, flexible, and team-oriented
  • Strong organizational, time management, and problem-solving abilities
  • Preferred: 1+ year of relevant experience in geriatrics, internal medicine, ICU, emergency medicine, or hospitalist settings
  • Nurse Practitioner license and DEA certification required
