*Summary*
The Acute Care Advanced Practice Provider will function as a healthcare provider, with physician oversight, who will work in collaboration with a broad multidisciplinary health team. This position will provide comprehensive care in the inpatient or outpatient setting, demonstrating a high degree of clinical expertise working with patients with acute and chronic illnesses, both hematologic and oncologic. This position will also be responsible for assessment, understanding of diagnosis, treatment, co-management, education, health promotion and care coordination for patients. The main goal of this position is to offer timely acute care in the outpatient setting to prevent emergency department (ED) visits and hospital admissions.
This position will offer support to both our Puyallup and Tacoma locations.
*Responsibilities*
* Supports and manages the complex needs of hematology and oncology patients requiring same day or timely acute care.
* Follows Value Based Care standards to help decrease overall costs to the patient, provide exceptional care, and improve clinical outcomes.
* Provide inpatient hospital coverage as needed or assigned.
* Communicate and work with patients’ primary MD to ensure continuity of care.
* Provides telephone support to field urgent calls from patients, hospitals, external providers, labs, etc.
* Participates in the call/clinic coverage schedule as needed.
* Sees same day acute care patients and informs the responsible provider if adjustments to the care plan are necessary to ensure optimal outcomes.
* Collaborates with physicians, nurses, and other healthcare professionals to coordinate patient care.
* Stays current with the latest advancements in hematology and oncology through continuous education and professional development.
* Contributes to the development and implementation of evidence-based practice guidelines.
* Serves as a resource for healthcare team members, providing expertise in hematology and oncology.
* Works closely with physicians, nurses, and other healthcare providers to adhere to established patient care plans.
* Participates in multidisciplinary meetings to discuss and coordinate patient care.
* May sometimes need to educate patients and their families on treatment plans, potential side effects, and available support services.
* Provides emotional support and guidance to patients and families throughout their treatment journey.
* Acts as a resource for colleagues, sharing knowledge and expertise to enhance the overall quality of care.
* Participates in quality improvement initiatives as needed.
* Responds to in-office infusion reactions and/or other adverse events for acute assessment and intervention as needed.
* Communicates changes in patient’s health status or urgent patient issues to the primary attending in a timely manner.
* Maintains accurate, complete, concise and timely documentation in electronic medical record system.
* Follows ethical principles regarding patient confidentiality, informed consent and unanticipated adverse outcomes.
* Considers cost effectiveness and risk/benefit analysis when making decisions about patient care.
* Complies with all company policies and procedures
* Performs other duties as assigned by management.

*Education/Experience *
* Minimum of two (2) years of experience in hematology and oncology preferred.
*Certificates and Licenses *
* Current, unrestricted license to practice as a Nurse Practitioner or Physician Assistant in the state of Washington.
* Active Drug Enforcement Agency (DEA) and National Provider Identifier (NPI) numbers, as well as prescriptive authority.
* Certification in Oncology or Hematology is highly desirable but not required.

Job Type: Full-time
Pay: $120,000.00 – $175,000.00 per year
