We are a fast-growing provider delivering innovative medical care to long-term and post-acute patients nationwide.

We seek a full-time on-site Nurse Practitioner or Physician Assistant to provide care in Skilled Nursing Facilities in Cincinnati, Ohio. No weekend, night, call, or holiday shifts.

Responsibilities include conducting health assessments, developing care plans, managing chronic and acute conditions, ordering diagnostics, prescribing treatments, educating residents and families, collaborating with interdisciplinary teams, and maintaining accurate documentation.

Requirements: Current Ohio licensure and board certification, geriatric care experience preferred, strong communication skills, proficiency with EMR systems, and ability to travel as needed.
