This full-time position involves providing primary and urgent medical care at a clinic and via telehealth, primarily serving patients with complex health needs, including older adults and those with multiple chronic conditions.
Candidates must be licensed in Virginia as a Family Nurse Practitioner or Physician Assistant, with autonomous practice preferred. Requirements include current licenses, DEA, Medicare, Medicaid numbers, and certifications such as BLS/CPR and ACLS within 30 days.
Responsibilities include managing diverse medical cases independently under medical director oversight, performing procedures like minor suturing, and addressing social determinants of health.
