The Licensed Mental Health Professional will be a key member of an innovative Safe Options Support (SOS) team, providing comprehensive care to homeless and subway-dwelling individuals in the community.
The multi-disciplinary team includes a Team leader, Licensed Clinicians, Care Managers, and Peer Specialists, utilizing the evidence-based Critical Time Intervention model.
Responsibilities include street outreach, coordinating needs before and after housing placement, enhancing daily living skills, offering supportive counseling, and advocating against discrimination and healthcare barriers.
The role emphasizes member choice, harm reduction, non-coercion, and person-centered care. The team supports participants post-housing to promote stability and independence.
This field-based position requires flexible hours, periodic on-call coverage, and travel within three counties. It offers full-time hours, competitive pay, and benefits such as health insurance and retirement plans.
