Job Description

The Licensed Mental Health Professional will be a key member of an innovative Safe Options Support (SOS) team dedicated to providing comprehensive care to street homeless or subway-dwelling individuals in the community.

The multi-disciplinary team includes a Team leader, Licensed Clinicians, Care Managers, and a Peer Specialist, utilizing the evidence-based Critical Time Intervention model.

The clinician will conduct street outreach, coordinate needs before and after housing placement, enhance daily living skills, offer supportive counseling, and advocate against discrimination and healthcare inequities. Core principles include member choice, harm reduction, flexibility, and person-centered care.

Post-housing, the team supports participants for several months to promote stability and independence. The role involves fieldwork, periodic on-call hours, and flexible scheduling.

This position offers a rewarding opportunity to impact community healthcare and improve lives of homeless individuals across three counties. Requirements include a master’s degree or higher in social work, counseling, nursing, or psychology, with licensure or plan to obtain it. Travel within the service area is expected. The role is full-time with benefits, based in Norwich, NY, and requires relocation before starting.
