Job Description
We are seeking a compassionate Family Nurse Practitioner to provide comprehensive primary care across diverse populations, including pediatrics, geriatrics, and adults.
Perform physical exams, assessments, and develop individualized treatment plans. Manage acute and chronic conditions, perform procedures like suturing and IV infusion, and support diagnostic evaluations. Coordinate care activities such as discharge planning, case management, and health coaching. Collaborate with multidisciplinary teams in various settings, including outpatient clinics, hospitals, and home care. Participate in telehealth services, educate patients on health topics, and maintain accurate documentation. Require current licensure, experience in primary care, proficiency with procedures, and knowledge of healthcare regulations and EMR systems. This part-time role offers $75,000-$85,000 annually for approximately 25 hours/week, working both in person and across multiple healthcare environments.
