Job Description

This role involves conducting in-home and virtual health assessments for Medicare, Medicaid, and commercial members to improve clinical outcomes and promote member engagement.

Key responsibilities include comprehensive evaluations, medication reviews, and patient education in a flexible schedule from 8am-8pm, Monday through Sunday.

Offers competitive pay ($100-$135/hour), completion bonuses, mileage reimbursement, and employer-provided malpractice and collaborative physician support.

Qualifications include an active nursing license, board certification as a Nurse Practitioner (Family, Adult, Gerontology, or Emergency Medicine), current CPR, and compliance with vaccination requirements. Prior home health experience, strong communication skills, and knowledge of Medicare/Medicaid are beneficial.

This position is based in Douglas County, OR, requiring relocation and in-person visits, with options for telehealth. Bilingual skills are a plus.
