Job Description

Seeking a compassionate MSW Social Worker to provide psychosocial assessments, counseling, and resource coordination for hospice patients and families.

  • Conduct assessments addressing emotional, social, financial, and spiritual needs.
  • Offer individual, family, and group support for grief and anxiety.
  • Create care plans collaboratively with the interdisciplinary team.
  • Connect families with community resources and support services.
  • Respond to crises and facilitate advance care planning.
  • Provide bereavement support post-loss.
  • Maintain accurate documentation and ensure regulatory compliance.
  • Participate in team meetings and community outreach.

    • Qualifications include a Master’s in Social Work, experience in hospice preferred, strong communication skills, and familiarity with community resources. This part-time, PRN role offers flexible scheduling, competitive pay, and the opportunity to make a meaningful impact in end-of-life care.
