Job Description

Seeking a compassionate MSW-level Hospice Social Worker to provide psychosocial assessments, counseling, and resource coordination for patients and families facing end-of-life challenges.

  • Conduct thorough assessments of emotional, social, financial, and spiritual needs.
  • Offer individual, family, and group support to address grief, anxiety, and psychosocial issues.
  • Develop care plans collaboratively with the interdisciplinary team, ensuring holistic care.
  • Connect families with community resources, assist with advance care planning, and provide bereavement support.
  • Respond to crises, maintain accurate documentation, and participate in team meetings.
  • Ensure compliance with regulations and engage in community outreach.

    • Qualifications include a Master’s in Social Work, experience in hospice or palliative care preferred, strong communication skills, and a commitment to compassionate care. This part-time, PRN role offers flexible scheduling, a supportive environment, and competitive pay, with work based in Eau Claire, WI. Candidates must be able to commute or relocate before starting.
