The LCSW-Social Worker for Case Management provides psychosocial support to patients with complex needs, collaborating with the healthcare team and community agencies to ensure safe discharge planning.
Offers a $3,000 sign-on bonus and is a full-time position.
Requires a Master’s Degree in Social Work, LCSW license, and current BLS certification.
Preferably has one year of supervised clinical experience in a medical setting.
Responsibilities include assessment, crisis intervention, referral, and care management.
Utilizes knowledge of psychodynamic and crisis theories, community resources, and brief counseling techniques.
