Job Description

The LCSW-Social Worker for Case Management provides support to patients with complex psychosocial needs, collaborating with healthcare teams and community resources to ensure safe discharge planning.

  • Offers comprehensive assessment, referral, crisis intervention, and care management services.
  • Assists patients and families in understanding and coping with emotional and biopsychosocial challenges affecting hospitalization and care.
  • Works as part of a multidisciplinary team to facilitate seamless care transitions.

    • Essential requirements include a Master’s Degree in Social Work, LCSW license, and BLS certification. Preferably, candidates have one year of supervised clinical experience in a medical setting.

    Offering a $3,000 sign-on bonus, compensation ranges from $40.00 to $48.10 per hour, with benefits such as health, dental, vision, life insurance, flexible spending, and paid time off. The role is full-time, in person.
