Post Acute Nurse Practitioner/ Physician Assistant – Hyannis, MA

Job Description

We are seeking a full-time Nurse Practitioner or Physician Assistant to perform primary care visits in Assisted Living Facilities in Hyannis, MA, and nearby areas.

Provide primary care, medication reconciliation, and annual wellness visits five weekdays.

Examine patients, review medical records, order diagnostic tests, diagnose conditions, and propose treatment plans.

Prescribe medications, maintain accurate records, and educate patients and families about health conditions and prevention.

Oversee patient health, promote wellness, and stay updated through professional development and research.
