Seeking a Full-Time Nurse Practitioner or Physician Assistant to provide primary care in Assisted Living Facilities in Hyannis, MA, and nearby areas.
Responsibilities include conducting exams, managing medical records, ordering diagnostic tests, diagnosing conditions, prescribing medications, and educating patients and families.
Provide annual wellness visits, medication reconciliations, and ongoing health oversight five weekdays.
Offer comprehensive care in patients’ residences, with access to an online portal for updates.
