Seeking a mission-driven Psychiatric Provider to deliver integrated, collaborative mental health care to medically complex and socially vulnerable patients.
Responsibilities include
Managing behavioral health for a designated cohort within a modified Collaborative Care Model Providing direct patient care, including diagnosis, treatment planning, and tele-psychiatry Collaborating with interdisciplinary teams, offering education, and supporting medication management, including substance use disorder treatments Facilitating care transitions, crisis intervention, and community outreach Qualifications include
Licensed, Board-Certified Psychiatrist in the applicable state Experience with addiction medicine preferred Strong communication skills, teamwork, and a passion for serving underserved populations Work is in-office in a collaborative, multidisciplinary environment focused on holistic, community-oriented care.
