Job Description
Seeking a licensed Nurse Practitioner or Physician Assistant passionate about senior care and innovation.
Provide outcome-based, comprehensive healthcare to chronically ill seniors within a team setting, collaborating with physicians, nurses, and other professionals.
Perform health assessments, manage patient care, order and interpret tests, and initiate emergency responses under supervision.
Maintain documentation, participate in quality improvement, and support care coordination, including 24-hour delivery.
Offer patient education and support to promote safety, independence, and well-being.
Requirements include valid licensure, experience with elderly populations, CPR/First Aid certification, a clean driving record, and ability to perform physical tasks.
