Nurse Practitioner: in home primary care, senior care

Job Description

We are seeking a part-time Nurse Practitioner to lead care teams and deliver in-home primary care to seniors in assisted living and long-term care facilities across Wisconsin and Minnesota.

Responsibilities include guiding clinical decisions, developing care plans, and collaborating with a supportive clinical team. This role offers flexible scheduling, competitive compensation, and potential growth into full-time or management positions.

Qualifications:

• Graduate of an accredited NP program with valid NP license and DEA certification in Wisconsin (MN is a plus)
• Experience in primary care, geriatrics, or in-home care preferred
• Strong communication, interpersonal, and customer service skills, especially with seniors and disabled individuals
• Valid driver’s license and auto insurance required
• Ability to work independently and align with mission-focused care
