Job Description

The Wound Care Nurse Practitioner (NP) reports to the Director of Nursing and supervises nurses and medical assistants.

  • Provides specialized wound care in home, nursing home, and assisted living settings for chronic, pressure, surgical, and complex skin wounds.
  • Performs assessments, develops care plans, educates patients/caregivers, and collaborates with healthcare teams to promote healing.
  • Requires a Master’s or Doctoral degree in Nursing, current NP licensure, and Wound Care Certification within 6 months.
  • Minimum 2 years of healthcare experience; active patient contact preferred.
  • Must be physically capable, adaptable, and have reliable transportation for travel to patient locations.

  • Essential functions include wound assessment, treatment, supervision, documentation, and interdisciplinary coordination. The role demands strong clinical skills, interpersonal communication, and flexibility in various environments.
