Job Description

Join a dedicated healthcare team providing patient-centered care in post-acute facilities. As an Advanced Practice Provider, you will manage and coordinate patient care, conduct rounds, communicate with staff, and document visits accurately. The role includes comprehensive training, shadowing, and oversight by a Medical Director to support your professional growth. Responsibilities include delivering exceptional patient care, collaborating with nursing and physicians, scheduling tests, and maintaining accurate records. Candidates should possess excellent communication, empathy, organizational skills, and the ability to work independently and as part of a team. Preferred experience includes geriatrics, internal medicine, ICU, or emergency medicine. A Nurse Practitioner license and DEA certification are required.
