Job Description

We are seeking dedicated Weekend Providers, including Family Nurse Practitioners (FNPs) and Physician Assistants (PAs), to join Hometown Health Clinic.

  • Provide high-quality urgent care through thorough assessments, individualized care plans, medication management, and patient education.
  • Maintain accurate records using systems like Epic and deliver care for infectious diseases and pediatric populations.
  • Collaborate within a multidisciplinary team and stay current on healthcare practices.

    • Qualifications include a Master’s degree, current FNP or PA-C certification, and experience with electronic health records. Strong communication skills and a commitment to patient-centered care are essential.
