Job Description

Seeking an experienced Family Nurse Practitioner (FNP) for outpatient mental health services in West Tennessee long-term care facilities.

  • Provide mental health assessments, diagnosis, and treatment for residents with conditions like depression, anxiety, Alzheimer’s, PTSD, Parkinson’s, schizophrenia, and bipolar disorder.
  • Offer counseling, medication recommendations, and collaborate with LTC staff and medical directors to ensure comprehensive care.
  • Utilize screening tools and clinical interviewing to develop individualized treatment plans and monitor patient progress.
  • Maintain detailed records and work independently or part of a team.
  • Requirements include current FNP licensure, knowledge of mental health disorders, excellent communication skills, and ability to travel within the region.
