Job Description

The Advanced Practice Provider (APP) provides comprehensive outpatient primary care across all ages under supervision, including evaluation, diagnosis, treatment, and documentation.

They coordinate medical, behavioral health, nursing, pharmacy, and ancillary services within a Medical Home model to optimize outcomes and cost-effectiveness.

Key duties include developing treatment plans, prescribing medications, interpreting diagnostics, patient education, and participating in after-hours call rotation.

The APP fosters a patient-centered, evidence-based approach, ensuring professionalism, confidentiality, and compliance with regulations. They collaborate with multidisciplinary teams to deliver personalized care and support quality improvement initiatives.

Qualifications include a Master’s degree in Physician Assistant Studies or Nursing, valid licensure, certifications (e.g., DEA, BLS, ACLS), and outpatient experience, preferably in Family Medicine. Experience with EHR systems and team collaboration is essential. The role is full-time at a clinic in Prineville, OR, with benefits and a requirement to relocate prior to starting.
