Job Description

The Advanced Practice Provider (APP) provides comprehensive outpatient family medicine care under supervision at the Prineville Clinic.

  • Responsibilities include evaluation, diagnosis, treatment, and documentation of all ages, coordinating services within a Medical Home model to optimize outcomes.
  • Creates treatment plans incorporating behavioral health, pharmacy, therapy, nutrition, and surgery; monitors ongoing care and refers to specialists as needed.
  • Practices evidence-based medicine, collaborates with multidisciplinary teams, and participates in after-hours call rotation.
  • Maintains professional standards, ensures documentation accuracy, and adheres to policies and regulations.
  • Requires a Master’s degree in Physician Assistant Studies or Nursing, active licensure, relevant certifications, and outpatient experience, preferably in Family Medicine.
  • Full-time role with a minimum of 40 hours/week, including scheduled call duties; benefits include health, dental, vision, retirement, and paid time off.
  • Location: Prineville, OR; relocation required before start.
