Job Description
The Advanced Practice Provider (APP) provides comprehensive outpatient primary care under physician supervision at the Prineville Clinic.
Responsibilities include evaluation, diagnosis, treatment, documentation, and coordinating services within a Medical Home model. Creates integrated treatment plans incorporating behavioral health, pharmacy, nutrition, and specialty referrals; monitors ongoing care and refers patients as needed. Works a minimum of 40 hours/week with scheduled patient contact and participates in shared call rotation, including evenings, weekends, and holidays. Collaborates with multidisciplinary teams to deliver evidence-based, personalized care and supports quality improvement initiatives. Maintains licensure, certifications, and compliance with legal and institutional standards. Requires a Master’s degree in Nursing or Physician Assistant Studies, valid licensure, and relevant experience in outpatient family medicine.
