The Advanced Practice Provider (APP) provides outpatient medical care across all ages under supervision, focusing on diagnosis, treatment, documentation, and care coordination within a Medical Home model.
Location: Prineville, OR, at the St. Charles Family Care Clinic. Schedule: Minimum 40 hours/week with at least 36 hours patient contact; shared call rotation including evenings, weekends, and holidays. Role: Collaborates with a multidisciplinary team, incorporates behavioral health, pharmacy, and other services into treatment plans, and monitors ongoing patient care. Responsibilities: Practice evidence-based family medicine, prescribe medications, interpret tests, educate patients, and ensure documentation compliance. Qualifications: Requires a Master’s in Physician Assistant Studies or Nursing, valid licensure, certifications, and outpatient experience; preferred Doctorate and Oregon licensure.
