Job Description

This position is for an experienced Hospice Social Worker dedicated to providing psychosocial assessments, counseling, and crisis intervention for patients with terminal illnesses and their families.

  • Conducts psychosocial evaluations, assesses family dynamics, caregiver abilities, and cultural needs.
  • Provides education on advanced directives, offers resource referrals, and facilitates communication between patients, families, and community agencies.
  • Maintains clinical documentation, participates in care planning, and collaborates with the interdisciplinary team.
  • Assists with end-of-life planning, legal, and financial arrangements, and supports quality improvement activities.


    • This full-time or part-time role offers competitive pay ($70,000–$80,000/year), benefits, and requires in-person work in Santa Fe, NM, with relocation assistance. It provides autonomy within a supportive team environment.
