Job Description
This position is for an experienced Hospice Social Worker dedicated to providing psychosocial assessments, counseling, and crisis intervention for patients with terminal illnesses and their families.
Conducts psychosocial evaluations, assesses family dynamics, caregiver abilities, and cultural needs. Provides education on advanced directives, offers resource referrals, and facilitates communication between patients, families, and community agencies. Maintains clinical documentation, participates in care planning, and collaborates with the interdisciplinary team. Assists with end-of-life planning, legal, and financial arrangements, and supports quality improvement activities. This full-time or part-time role offers competitive pay ($70,000–$80,000/year), benefits, and requires in-person work in Santa Fe, NM, with relocation assistance. It provides autonomy within a supportive team environment.
