Job Description
The Licensed Clinical Social Worker (LCSW) provides evidence-based psychotherapy, including individual, group, and family counseling, within an integrated primary care team.
They conduct assessments, develop treatment and self-management plans, and offer crisis intervention and behavioral health referrals.
Working closely with clinic staff and external partners, they support patients’ emotional and physical well-being, document care in the EHR, and participate in team meetings, trainings, and quality improvement initiatives.
The role requires cultural competence, independent judgment, and adherence to operational standards.
