Seeking a full-time Mental Health Professional (LPCC or LMFT) in Waverly, MN, offering an annual salary of $62,005-$86,158.

  • Assist primary care providers in identifying and treating mental health and psychosocial issues.
  • Assess referred patients through brief consults and collaborate with the primary care team to manage chronic emotional and health problems.
  • Help detect at-risk patients and develop prevention plans.
  • Evaluate care plans, educate patients and families, and monitor behavioral health programs for improvements.
  • Participate in meetings, quality initiatives, and committees, as well as peer evaluations.
  • Requirements include a Master’s in Behavioral Sciences or related field with 4000+ hours of supervised clinical experience and current licensure as LPCC or LMFT.
