We are a primary and urgent care facility in Middletown, NY, serving multiple locations since 1984.

  • Our dedicated team includes experienced physicians, nurse practitioners, and PAs committed to providing comprehensive, quality care in a warm environment.
  • We have an immediate full-time opening for a Physician Assistant or Nurse Practitioner with at least one year of pain management experience.
  • The role involves seeing patients for initial procedures and follow-up visits alongside a pain management team.
  • We offer a competitive salary ($60-$65/hr), flexible schedule, benefits (including health, dental, vision, life insurance, CME reimbursement, malpractice, and retirement options), and fast credentialing.
  • Work schedule is Monday to Friday, in person, with approximately 40 hours weekly.
  • Preferred candidates have pain management experience and are licensed in NY.
