Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.
Deliver care to a range of patients from short-term rehabilitation to complex geriatric cases, aiming to improve access, remove barriers, and achieve optimal clinical outcomes.
Participate in a structured care model that enhances facility cultural, clinical, and financial performance.
Provide face-to-face primary care, engage in educational sessions, and collaborate with nurse practitioners and physician assistants.
